Leao was let through on goal by the Brazilian’s second touch, and the Portugal forward fired an attempt through the legs of opposing goalkeeper Juan Musso to put Milan ahead.
While the hosts’ defence had a few nervous moments, Hernandez’s magnificent goal calmed everyone down.
Stefano Pioli’s team has now gone 15 games without losing in the league.
Inter would have won the title if they had not defeated Cagliari, but Simone Inzaghi’s team prevailed to take it down to the wire.
Inter took the lead through Matteo Darmian, and Lautaro Martinez increased the advantage with a precise finish.
Cagliari – and Milan – had a chance thanks to Charalampos Lykogiannis, but Martinez’s late dinked goal clinched the victory.
